The UNC Department of Urology is pleased to offer two options to verify the clinical training of our residency graduates and fellowship graduates.

OPTION 1 - STANDARDIZED DEPARTMENT VERIFICATION OF TRAINING FORM
Cost: $60

Standardized department verification of training form modeled on National Association of Medical Staffing Services best practices. Form information includes trainee full name and date of birth; training specialty; dates of training; program completion status; program accreditation; any relevant leave, remediation, and/or early dismissal from the program; and any additional comments, as necessary.


OPTION 2 - UNIQUE INSTITUTIONAL FORM
Cost: $100

The program will complete a unique institutional form upon request. Please send completed, signed forms to Aleasha_Crisp@med.unc.edu
