 The landscape is changing.  It is imperative that hospitals and communities work together to reach and service our patients most in need.  NCONA’s annual patient navigation conference will address best practices in bridging our health systems and communities.  This conference is for oncology nurse navigators, administrators, care coordinators, advanced practice nurses, patient navigators, community health workers, case managers, social workers, patient advocates, community ambassadors, lay health advisors, community educators, peer supporters and lay volunteer patient navigators.  Attendees will have the opportunity to network and make new connections with members in this exciting profession.  We look forward to seeing you!!

This session highlights successful community health worker programs and the potential strategies for linking their work with patient navigators.

Overview Tish Singletary, Project Specialist NC Division of Public Health

Presenter will discuss how to successfully integrate CHWs into health care teams, how to maximize the impact of these workers in the self-management of chronic disease, and how to strengthen their role as key emissaries between clinical services and community

Local Programs

Panelists will describe the roles/responsibilities of CHWs in their programs and the general health issues they address. Additionally, panelists will present the types of services provided and the populations served by the programs. Finally, speakers will share strategies for working with CHWs.
